The selected candidates will work closely with individuals requiring specialized care, conducting in-depth assessments, creating tailored care plans, and fostering independence through life skills and therapeutic activities. This role involves setting and pursuing comprehensive health, psychological, and social objectives to facilitate community integration and mentoring community support workers.
Key Responsibilities:
• Engage one-on-one with clients facing disabilities, dementia, and challenges associated with aging.
• Conduct assessments and create individualized care plans.
• Develop life skills and engage in targeted therapeutic activities with clients.
